Healthwatch Norfolk is working with NHS Norfolk and Waveney to find out if GP practice staff have used the Shared Care Record when treating you.

The record aims to ensure that different people who treat and look after you all have access to up-to-date information about you to help with your care.

We want to find out:

Have GPs or other practice staff used the Shared Care Record during appointments or care?

If you were asked to tell your story again, would you mention the Shared Care Record to them?

Share your experiences in the survey and these will be analysed for a report to NHS Norfolk and Waveney.
