Winter pressures care home pilot project Autumn 2018

A majority of people felt the Winter Pressures Care Home Pilot Project, which ensured regular contact with a GP or Nurse Practitioner, for residents of 27 North Norfolk care homes should be extended.

Acle Medical Partnership asked Healthwatch Norfolk to get some feedback about the Winter Pressures Care Home Pilot Project which provided additional support to 27 North Norfolk care homes by focusing on a continuity of care and regular contact with a GP or Nurse Practitioner over an eight week period.

Staff felt it had improved continuity of care and it seemed to work well subject to some adjustments and alterations.

A majority of people questioned (7 out of 10) felt it was a project they would like to see in place in the future.
