Are you waiting for sight loss support?

Are you living with sight loss and currently waiting for a diagnosis, or support for your condition?

We’re gathering feedback from people who are living with sight loss to find out how long people are waiting for diagnosis and if they are given any support while they are waiting.

We’re interested in hearing from anyone in Norfolk who has a vision impairment, or is caring for someone with a vision impairment. 

We want to know which healthcare services you’ve accessed, what the process of referral for further assessment and diagnosis has been like and the level of support you’ve received for managing your condition while you wait.

If you live in Norfolk and would prefer to do this survey with us over the phone, please call Healthwatch Norfolk on 0808 168 9669 and we will arrange a time to ring you back to complete the survey. 

You can also email [email protected] to receive the survey in alternative formats, including Easy Read and large print.

The survey takes 5-10 minutes to complete. Some questions are optional.

All responses will be anonymous. 

The survey responses will be collected and analysed by Healthwatch Norfolk and used as part of an independent evaluation of the support for people with sight loss.  The report will be publicly available on our website and may be used in other Healthwatch Norfolk communications.
