Share your views on the future of the Vulnerable Adults Service
NHS Norfolk and Waveney wants to hear your views about the future of the Vulnerable Adults Service Health Inclusion Hub.

NHS Norfolk and Waveney says it wants to make sure it gets the best value for the public money it receives. It has developed some options about how the hub could run in the future, and wants to get people’s views on them.
Vulnerable Adults Service Health Inclusion Hub
Based in Norwich, it brings together specialist healthcare professionals to provide enhanced medical support for people with a complex range of needs.
It helps those who are socially excluded, live chaotic lives, are at risk of having poor health, and may not be on electronic health systems. Many of the users are homeless.
The team give support for up to six months before integrating patients back into one of 20 GP practices with a plan for their ongoing help.
Officials are suggesting two options for its future:
A – Continue providing the service from its current base.
B – Create a team which will provide support to different communities across Norfolk and Waveney, and close the Norwich base.

What happens after I have shared feedback?
All the feedback must be received by 9am on April 22.
Each piece will be individually analysed by the Healthwatch Norfolk team and a full independent report published by mid-May.
This report will then be considered by the NHS Norfolk and Waveney Board.
